These packets contain all of the information from the Home On The Range Admissions/Referral Packet.  All packets are in Microsoft Word Format or Portable Document Format (PDF).  The forms in Microsoft Word may be directly typed into, e-mailed, or printed and then faxed or mailed to HOTR.

Health Tracks Form

The Health Tracks form is the only form not included as a Microsoft Word Document. The Health Tracks form is listed as an individual form and needs to be separately downloaded from the packets.

Educational Grant

If you were successfully discharged from Home On The Range, you may be eligible to apply for an education grant. This grant may be used for vocational, college or university enrollment. For more information and the application, please click on the Fraternal Order of Eagles Memorial Foundation Scholarship Forms that follow. The completed forms, application and necessary paperwork can be mailed to Home On The Range. Home On The Range will verify resident’s admission and discharge dates, and then send the packet to the Memorial Foundation. The forms follow:
